Medical Forms

Please note that there are new medical forms to be used beginning January 1, 2010.

The BSA Health and Safety Committee took on the challenge and the opportunity to establish one Annual Health and Medical Record
for all ages and known risks. It is simple and yet comprehensive enough to be used for both health and wellness evaluations
as well as emergency situations.

Major changes:
A health history is still sufficient for typical activities lasting less than 72 hours
(Parts A and C of the new form—similar to the old Class 1 form).

For activities lasting longer than 72 hours, a medical evaluation by a health-care provider is now required annually (Part B).

For high-adventure activities for which medical care may be delayed, restrictions based on standardized height/weight ratios are now mandatory.

Both medical forms are now valid for 12 calendar months.

There is no age restriction for these medical forms.  The forms and requirements are the same for scouts and adult leaders.

Parts A, B and C:

Every scout needs to submit a Parts A and C scout form completed by their parents within the past twelve months to participate in any activity lasting 72 hours or less:

Every scout needs to submit a Part B scout form completed by a certified and licensed healthcare provider within the past 12 months to participate in any actvity
lasting 72 hours or more or for any high adventure activities.
